    Service Application

    A Government-issued photo ID Required along with deposit fee of $59.77 must be paid with this application.

    New Tap Application

    A Government-issued photo ID Required, a Tap and deposit fee of $359.77 must be paid with this application. Service is required to be paid for 36 months continually before shut off can be requested.

    Tap fee ($300.00) is non-refundable once work is started.

    Leak Adjustment Request

    Leak adjustments will not be given for unusual usage due to leaking commodes, dripping faucets, outside faucets, hydrants, malfunctioning appliances, and similar situations that would be easily noticed by the customer.

    **All Leak Adjustments are subject to Water Board Approval.

    Payment Plan Request

    Please print off the form, fill it in as best as possible, and return it to the office by mail or in person. ** No Digital delivery is available for this form**

    In the case of a customer requesting an installment plan for the payment of a large bill due to a leak, all the terms of the plan must be met by the customer to avoid penalty charges on current bills.

    Deposit Trasfer Request

    Do you want to transfer the deposit on your account to someone else who is taking over the bill for your service address? (for example, your Son or Daughter taking over water account) Simple template for you to request a transfer of deposit, we will also accept a signed letter mailed to us or dropped off. Goverment- issued photo ID for original account holder is required for form to be valid; as well as service application (located in link above) and Goverment- issued photo ID of persons taking over account.

    ** PLEASE NOTE: ANY OUTSTANDING BALANCE MUST BE PAID

    IN FULL BEFORE TRANSFER, OR TRANSFER WILL NOT BE COMPLETED.**
